Urgent Care
- These are the most common visits for MACC and DHP same day clinic
- Sometimes you may have urgent care visit scheduled for your continuity clinic days
- goal: focus should be the acute issue, pick 1 or 2 issues to focus on that you can make acute changes with
- If you order labs/images, make sure to call the patient to follow up
- Try not to delve too much into chronic issues that needs to be addressed by PCP
Hospital Follow Up
- these visits are scheduled for patients who were recently seen in the ED or discharged
- goal: focus should be on hospital issue, medication reconciliation, repeat labs, and ensuring proper long term follow up with PCP/specialties
- there is usually a discharge summary from the admission that includes things to do at follow up visit
- many times you will need to have repeat labs. E.g. GI bleed in hospital needing repeat CBC at follow up
- patients may need medication restarted. E.g. Septic patient with BP meds on hold until follow up
PCP Establish Care
- These are new patient visits where you see them for the first time as PCP
- Main purpose is to get acquainted with the patient
- Figure out any big to dos from health perspective
- Acute issues ⇒ chronic health management ⇒ health maintenance ⇒ medicare wellness
PCP Follow Up
- These are the bulk of your visits during your clinics
